Clinics provided at the CHFC facility:

• Primary Care Clinic
• 1st Saturday of the Month Clinic
• Dental Clinic
• Eye Clinic
• Women's Clinic
• Mammography Clinic
• Osteoporosis Screening Clinic
• Podiatry Clinic
• Sports and School Physicals

Referrals are needed for the following specialties/services:

• Dermatologists
• Physical Therapy
• Orthopedic Clinic
• Counseling
• Psychiatric Counseling
• X-Rays and Laboratory Tests
• Heart Stress Tests

Medication Related Programs are available through the following:

• Prescription Assistance Program
• Local Participating Pharmacies
• In-House Pharmacy*
  
  * The CHFC pharmacy does not store and medical/dental professionals do not dispense, prescribe or provide controlled substances including narcotics, depressants and
  stimulants under Schedule II, III, IV and V of the Drug Enforcement Act.
