Medication Programs

Prescription Assistance Program

Long-term medications for three months at a time may be available directly from the Pharmaceutical Companies.
Not all medications are available.
To qualify:
- Patient MUST first be seen by the clinic doctor and have a green sheet completed that allows medications for one year.
- Patient MUST meet financial guidelines set by the pharmaceutical company.
- Patient MUST provide written proof of income before medications can be ordered.
- Patient MUST be seen at least yearly by a Community Health Free Clinic physician.
Each patient is encouraged to bring in medicine bottles of drugs they now are using.
The first prescription order generally takes 6-8 weeks to arrive. If unable to pick-up prescription in person, please provide written permission to authorized person.
Prescriptions are picked-up at the Prescription Assistance Office during Program hours.

ALL MEDICATIONS MUST BE REORDERED 30 DAYS PRIOR TO RUNNING OUT. PLEASE REORDER ON TIME!
Reorders will be taken over the phone IF your file is in order.

Prescriptions written on yellow or Blue forms in the Community Health Free Clinic (CHFC) are to be taken to either of the above locations.
Refills are not paid for by the CHFC.
No narcotics or Class 2 drug prescriptions will be written by CHFC professionals.
If you have a narcotic or Class 2 prescription written by a physician or dentist practicing outside the CHFC, we may be able to assist you in purchasing it from one of our participating pharmacies.

Some drugs are available in our in-house pharmacy.*

* The CHFC pharmacy does not store and medical/dental professionals do
not dispense, prescribe or provide controlled substances including narcotics,
depressants and stimulants under Schedule II, III, IV and V of the Drug
Enforcement Act.
