Referrals

Referrals are needed to use the following specialties/services:

Dermatologists

You must be examined by a physician at the Primary Care Clinic in order to be referred to a Dermatologist.
The Clinic staff will make the appointment for you.

Physical Therapy

You must see a physician at the Primary Care Clinic in order to be referred to a therapist.
Patient will schedule their own appointment with a Physical Therapist upon referral by the Primary Care Clinic professional staff.

Orthopedic Clinic

Patients must first be examined by a medical staff professional in the Primary Care Clinic in order to be referred to the Orthopedic Clinic.
If you have had X-rays taken, please bring them with you to the clinic. You will be able to take them with you for return to the lender after the orthopedist views them.

Counseling

You must see a physician at the Primary Care Clinic in order to be referred to a professional counselor in the Community Resources Service.
Physician's referral notes are used during the Community Resource Counseling visits.
These appointments are scheduled by the counselors themselves.

Psychiatric Counseling

This is available for those who are waiting for an appointment at another facility and have been referred by the Primary Care Clinic professionals.

X-Rays and Laboratory Tests

You must see a physician at the Primary Care Clinic in order to be referred for radiologist's services (x-rays).
Most x-ray studies are done by Mercy Medical Center or St. Luke's Hospital Radiology Departments.
Laboratory tests -- only when clinically necessary and ordered by medical/dental professionals in the CHFC clinics -- are available form Mercy Medical Center and St. Luke's Hospital as charitable gifts in kind.

Heart Stress Tests

You must see a physician at the Primary Care Clinic in order to be referred to the cardiologist.
Appointments will be arranged for you.
If it is necessary to cancel the appointment, please do so at least 48 hours prior to the appointment to safeguard additional appointments for uninsured patients.
